Data entered into this portal is encrypted and transmitted directly into our clinic's secure internal schedule. This data is governed by Tracker’s privacy protocols and is not stored on the public-facing website.
When you submit a "Returning Patient"
appointment request or use our "Contact Us"
form, we use Formspree to
securely filter and transmit your message.
Formspree acts as a courier; they process your
Name, Email, Phone Number, and Appointment
Preferences and forward them to our reception
team via email. Formspree does not sell your
